Hall of Fame Nomination Ballot

The Athletic Hall of Fame is designed to honor those individuals who, through their commitment, sacrifice, and dedication to the Palatine High School Athletic Program, have brought pride and distinction to our school community as either a player, coach, administrator, or active supporter.

Inductees are limited to those who have been graduated for at least five years and those who have retired from their athletic related positions.  Nominations may be submitted by any supporter of PHS athletics.  Forms can be submitted via the web below, or you may download a form in MS Word format here.

Forms must be received prior to April 15th for consideration for induction in the next school year.  Further information regarding the Hall of Fame can be obtained by contacting the Athletic Office at (847) 755-1770.
